(b) ID card and personal seal of applicant
Fee: NT$1500 for10 copies.

On legal holidays , please submit request to Juisheng Hospital and Kuohsiang Clinic.
In the case of suicide, murder, accidental death and where there is no medical certificate issued by a hospital, please report to local police station with request for a coroner's examination.
